How does SHCPS provide these FREE services?
We work with a third party program that contracts with each provider clinic/group we work with. This program pulls pharmacy data for all patients associated with each provider NPI associated with the clinic/group. From there, patients are screened for pharmacy care needs such as CMRs, gaps in therapy, and adherence. Those patients are sent to a platform where a SHCPS Clinical Pharmacy Specialist is ready to outreach the patient to make an intervention where needed. The contracted provider clinic/group receives the reimbursement directly from this program. SHCPS would then be paid from that reimbursement coming into the clinic. A service that pays for itself! It is a win-win-win! Providers don't have to do the busy work to show improved Medicare measures. Patients have optimized medication management. SHCPS gets paid for the work provided.

What do the provider clinics/groups have to do to get this service?

- Meet via phone, video, or in-person to discuss what SHCPS can do for your clinic to best meet your needs.
- Sign a business agreement between provider clinic/group and SHCPS. Ensure HIPAA compliance on both ends. This way SHCPS works with the clinic/group.
- Though not required, granting SHCPS access to EHR allows for comprehensive patient management. Without EHR, SHCPS can still complete patient care, but sending recommendations or consults would be a little more cumbersome via fax or phone. EHR allows for convenient provider/pharmacist communication as well as a more complete picture of the patient.
- Provide provider clinic name, contact person, phone number, fax number, address, and W9 (for reimbursement).
- SHCPS will submit all information for contracting. Once reviewed and approved, an electronic contract is sent to the clinic/group contact person for signing.
- Provide SHCPS a list of all provider NPIs in the clinic/group.
- SHCPS will start receiving eligible Medicare, Medicaid, or commercial patients within a few days.
- Reimbursement comes monthly to the provider clinic/group. Electronically send or write a check to SHCPS.
Are there other services that SHCPS can provide other than opportunities within OutcomesMTM(R)?
- Our pharmacists are residency trained, board certified clinical pharmacy specialists with over 20 years of combined experience in ambulatory care internal medicine.
- Collaborative Drug Therapy Management (protocol) agreements can be used to allow pharmacists to manage many disease states including heart disease, hypertension, dyslipidemia, anticoagulation, thyroid, and diabetes.
- These additional services are not paid for by third-parties and would require a fee-for-service discussion. However, "incident to" billing is available and may offset the cost of the pharmacist.
