Technology and Logistics
Telehealth Services
We know that space in a provider clinic in a commodity. We also know that patients prefer care delivery in the comfort of their own homes. We offer telehealth via video conferencing (doxy.me) and telephone.
Electronic Medical Records
Most provider clinics/groups grant SHCPS access, as a contracted business, to the EHR. This allows a more complete picture of the patient to provide patient-specific medication reviews and recommendations. This also allows SHCPS to communicate efficiently with the provider, if needed.
Patient Consults
Consults and pharmacy outreach initiated by OutcomesMTM(R) data are no charge to the patient. However, patients are able to speak with a pharmacist via appointment for a fee if they have specific questions or concerns about their medications. They do not need a referral, but referrals are welcome!
HIPAA and Compliance
Patients coming to SHCPS via OutcomesMTM(R) will be notified that SHCPS works with your clinic directly and, therefore, expect SHCPS to be in compliance as a contracted employee/business group.
Patients coming via referral will fill out online HIPAA and Patient Intake Forms (found on this website under appointment scheduling).
